  • Leaders Life - Term to 100 Life Insurance

    Leaders Life can be added or changed anytime of the year. As a new hire, it is important to review this Life Insurance option, because you will have limited health underwriting.

     

    New Hires can purchase the following Contingent Guaranteed Issue amounts:

    Employee $100,000

    Spouse $50,000

    Child $50,000

    Grandchild $50,000

    (only answer the CGI questions on the first page of the application)

     

    Existing employees must apply for coverage by answering all health questions during the application process.

    24 Hour Coverage

    In the event of an unexpected injury, Aflac can help protect your personal finances. We provide individuals and families affordable insurance that helps with expenses that my not be covered by major medical insurance.

  • The rates shown below are an example and do not in any way constitute a guarantee in coverage. Please visit with your company's dedicated agents Ricky DeFalco or Cindy Hawkinson to get a specific quote, to add/change current coverage or to apply for new coverage.

     

    (Additional Riders are available on most plans Aflac plans)

     

    Plan changes can be made during your open enrollment, unless you have a qualified life change event.

    Flexible Spending Account (FSA)

    Every employee is eligible to set up an FSA account through SFS.

     

    Plan Start: Sept 1st

    After the plan year starts, you cannot change deductions without a Qualified Life Event.

    Maximum Contribution: $3200 per family (2024)

    Rollover Maximum: $610 (2024)

    Free FSA Debit card will be sent.

    Dependent Care Reimbursement Account (DCRA)

    Maximum Contribuition: $5000

    Benefits are reimbursed after costs are incurred.

    Plan Start: Sept 1st

    Health Savings Account (HSA)

    REQUIRES A QUALIFIED HEALTH PLAN

     

    You can opt in or out of this plan throughout the year.

    Maximum Contribution Annually: $4,150 ind/$8,300 family (2024)

    Age 55+ can add another $1,000 (catchup contribution) to your maximum.

     

    These Funds Rollover and are held in a Savings Account that you control.

    Free HSA Debit card will be sent.
